My name is John H Gonzalez. I was booked for PAL flight out of Cebu bound for SFO via Mnla last September 6th 2021. I had an RT-PCR rest done at BioPath in Cebu city on September 4th, 2021 at around 0900hrs and received the result at 1800hrs on the same day. The result proved positive. I advised my Travel Agency ( City Tours in Los Angeles ) of this development where the said Agency cancelled my passage for rebooking. I have been in isolation from the 4th to the present ( today is the 10th). According to D O H personnel, if I donāt show any symptoms on the 14th of September, I shall be given a certificate of recovery. I have also conformed to vaccination requirements of two (2) doses of Astra Zeneca vaccine given on May 18th and July 27th of this year from my hometown which is supported by a Bona Fide card filled by the Rural Health Unit where I live. At this juncture, please inform me of any and other requirement that I may need to complete my trip to San Francisco. I look forward to your answer to this query.
